Community Counseling Centers of Chicago (C4) was founded in 1972 to help people released from psychiatric hospitals into Uptown & Edgewater heal in the community. Since that time, C4’s service area has expanded to rekindle hope for individuals and families on Chicago’s north and west sides, remaining committed to community-level, system-focused interventions; C4 always seeks to avoid restrictive levels of care and maximize families’ agency to lead their recovery, allowing them to heal in their homes and community.

C4 is the leader in building hope and breaking down barriers for Chicago’s systematically disadvantaged communities through the delivery of effective, empirically supported, and trauma-informed mental health services. Treatment is a partnership between the individual, C4 staff, and a network of family and friends; this supportive atmosphere enables healing in a safe, trusted environment marked by professionalism and skill.

Our Services

Children and Family 

Mental health experts provide child-centered and family-focused support that promotes the positive cognitive, social, and emotional development of children and adolescents; including school-based, trauma response, and violence prevention specialized therapeutic programs

Rapid Response & Community 

Our rapid response teams of mental health professionals provide crisis intervention services for children, adolescents, and adults in severe psychiatric distress;

Counseling & Support 

C4 helps clients develop better mental health self-management, coping, and relapse prevention skills and works to connect them with resources in the community;

Medical Services

Our community mental health centers also provide an offering of medical services for children, adolescents, and adults, including partnerships and on-site care.
