 Training Description

Showcasing an Array of Hats Needed to Lead in Case Management: What essential skills will your hat display? Case Management is a reporting technique to help professionals assess the needs of the clients they serve to improve their overall quality of life; managers coordinate, monitor, and evaluate to maintain successful records to ensure that clients are transitioning to their next level of greatness. In this workshop, participants will go through different stages: screening clients to meet your program’s needs, assessing what support is needed to help reach targeted goals, determining any risk involved (sharing testimonials), planning through short and long-term goals with your team and facilitating access to community resources, implementing (execution/coordinate and monitor services) goals from team meetings and client one-on-one sessions, and direct reporting (review, evaluations, monitoring, follow-up and provide any emergency crisis services if necessary).
